Hourly care

Hourly care is customized for individuals to have care at the time of day they need it most for a duration of time that is appropriate to assist with their care needs. Hourly care can range from a one hour a day up to 24 hours a day with a team of caregivers working longer shifts if a client’s level of care requires it. Pinnacle Home Care Solutions has a 4-hour minimum for hourly care.

Live-in care

Live-In care is appropriate for individuals that require the presence of a caregiver on site 24 hours/day. The caregiver lives in the client’s home. People with mobility issues or memory impairment are usually the best candidates for live-in care. On average, the cost of live-in care is equivalent to 10 hours of hourly care with some additional ancillary costs such as grocery items for the live-in caregiver.

Supplemental staffing

Pinnacle Home Care provides supplemental staffing for local hospital, rehab facilities and assisted living communities to support them with their coverage needs when staffing shortages occur. We act as a partner to the communities we contract with to provide seamless coverage in accordance with the professional standards and expectations of the communities we support.

Clinical & case management visits

To ensure our clients are receiving the quality of care they need, our nurse care managers conduct on-going case management visits for assessments, care plan coordination and personnel supervision. Visits are conducted face to face, via phone or by video conference.

Transitional/short term care

Transitional care can be highly effective for someone whose independence may be compromised due to a surgery, fall, stroke or anything else that may limit their ability to care for themselves. Transitional care helps an individual for a short time until they fully recover and regain their independence.

Respite care

Respite care can be a short-term solution to give a primary caregiver a temporary break from their caregiving responsibilities. Respite Care can be an effective preventative from “caregiver burnout”. It allows the primary caregiver an opportunity to have some relief without compromising a loved one’s care needs.
